Terms: Credit is applied once per child and is non-transferable. Screening credits are valid for 60 calendar days (8 weeks) from the date of the screening appointment. Credits expire automatically after 60 days and cannot be extended.

Cancellations or rescheduling with less than 24 hours’ notice will be charged the full screening fee. No-show appointments are charged the full screening fee.

This appointment is not eligible for insurance forms or letters.

Screening does not replace a full standardized assessment.

No diagnosis, treatment plan, formal scoring, or report is provided.

If immediate concerns are identified, the clinician may recommend booking a full evaluation sooner than the 60-day window.
